Add an Enrollment Form to your Experience
Enrollment Forms enable employees to enroll themselves or others into Dado experiences, without needing access to the Dado admin interface. These can be used in a range of different ways:
- A form for recruiters to complete, to kick off the onboarding for a newly hired worker
- A form for managers or HRBPs to fill out, to notify HR that their employee has just handed in their resignation, so the offboarding process can begin
- A form that enables employees to enroll themselves in an experience, such as self-guided training, requesting leave or exploring benefits policies that are relevant to them
- A form to accept applications to your mentorship program

How enrollment forms work
When you add an enrollment form to your experience, you can specify what fields are on the form and what approval is required before someone can be enrolled.
Every enrollment form has a unique link, which can be shared with anyone within your organization – or outside! You do not need to have access to the Dado admin app to use the enrollment form. You can use enrollment forms to enable employees to enroll themselves in experiences; or to enroll other people.
As soon as someone submits the form, one of two things will happen, depending on the approval settings you've chosen:
- The person whose details were submitted is instantly enrolled in the experience
- An approval is requested of the submission. If approved, that person will be instantly enrolled. These approvals can only be done from within the Dado admin app.
Setting up an enrollment form
Step 1: Add the enrollment form to your experience
On the View Experience page, click the dropdown icon on the blue Enroll button, that appears in the top right. Select Enrollment Form.
A new window will open with the settings of your enrollment form. You can see a preview of the form on the right hand side of the window.
Step 2: Decide on your approval settings
There are two options to select from here:
- Require admin approval: when you select this, each form submission must be individually approved from within the Dado admin app before the individual is enrolled.
- Use this setting for sensitive experiences, those where you'd like control over who gets enrolled, or those where the timing of someone starting the experience is important.
- Auto-enroll: when you select this, the individual will be enrolled as soon as the form is submitted, with no approval step.
- Use this setting for non-sensitive experiences, or while testing an experience
Step 3: Add intro text
The introductory text field is displayed at the top of your enrollment form. It can be a helpful guide if someone who isn't on your team is going to use the form. Consider explaining:
- when they should and should not use the form. EG if this is an onboarding experience, should they be enrolling all new joiners, or only full time employees?
- what will happen when they submit. EG, will the employee be enrolled instantly or only after approval?
Step 4: Select your fields
- First choose which email field appears on the form.
- Either work or personal email address is required, as Dado uses these to uniquely identify your employees
- Then select which other fields you would like to include on the enrollment form
- Select the field from the dropdown of existing Dado fields. If you need a new field here, first create it as a custom field, then you can select it in the dropdown.
- Once you've selected a field, you have the option to change the display name that will appear on the form – for instance, you could relabel the Department field, so it displays as "Division"
- All fields on the form are required. If a field is only needed in some circumstances, we suggest you include instructions in the display name, such as "Enter N/A if not applicable"
By default, all fields are open text, with the exception of date-type fields, and user relationship fields (eg Manager). If you'd like to make a field a single select, select Restrict answer choice
- You have two choices of how to restrict answers: either enforce that only existing values can be set, or specify the values yourself
- Use List existing field values for datapoints you already have for existing employees, such as Department, Team or Seniority Level
- Use Enter a list of allowed values for datapoints where your existing employee data doesn't have a full list of acceptable choices, eg for fields that are only used within this experience.
- You have two choices of how to restrict answers: either enforce that only existing values can be set, or specify the values yourself
When selecting fields for your Enrollment Form, think about which fields are used in your Experience to define Target Audiences, and which fields you insert as variables.
Unless those datapoints are set by Sub-Tasks during the Experience, or are coming from an Import Source, you should include them in your Enrollment Form.
Step 5: Save and activate your form
When you're happy with your form, click the activate toggle at the top, and then the save button at the bottom right. Please note: you can only activate a form when the Experience itself is active.
Once activated, you'll see a link for your experience, which will appear on the settings page, and on the View Experience page:
Editing an existing enrollment form
It's easy to make changes to an existing form, whether that's tweaking the intro text, adding/removing fields or changing the approval settings.
- On the View Experience page, click the dropdown icon on the blue Enroll button, that appears in the top right. Select Enrollment Form.
- In the settings screen, make your changes, then hit save in the bottom right
- The enrollment form will update with your changes immediately. No past enrollments will be affected.

Who can access the enrollment form
The Enrollment Form can be accessed by anyone with the link, even if they don't have an account in the Dado admin app.
As soon as your enrollment form is activated (see Step 5 above), you can find this link on the View Experience page:
Approving enrollment form submissions
If you have selected that all form submissions require admin approval, every time the form is submitted someone will need to approve the submission, before that individual is enrolled.
When there is a submission needing for approval, an orange banner will appear on the View Experience page:
Clicking on Review will open a screen showing all the submissions:
- Click on a particular submission to see the full details of what was submitted on the form.
- By clicking on the submission, tou can amend anything on the form before approving this submission.
- Click the Enroll button to instantly enroll that individual
- Click the Reject button to reject those enrollments
Guide to the statuses of enrollment submissions
Pending | This submission needs to be reviewed and either approved or rejected |
Rejected | This submission has been rejected. You cannot undo a rejection; but you could re-submit the form for this individual. |
Enrolled | This submission has been approved and the individual enrolled. |
Invalid | This submission does not meet the requirements for enrollment. |
Error | Something has gone wrong with this enrollment. Please contact Dado customer support. |
